Important New Policy Notice!
Dear Parents of Swimmers,
The following Waiver/Release Form must be signed in order to participate in our swim lessons subject to our new insurance policy. If you choose not to sign the form, we will unfortunately have to ask you to leave the program on that day, as you will not be covered under our insurance policy. This policy will be strictly enforced. Please print and sign this form and fax it to (212) 828-8842 or bring it to your first day of swim. If you choose to fax this form, call the office to confirm that we have received it.
HEALTH CLUB RULES: All individual health club and board of health rules will be strictly enforced. Please see your TMTTW Director or call the office if you have any questions.
NO EATING: Please do not allow children to eat at least one hour prior to their lesson. If your child gets sick more than once you may be asked to leave the program to maintain the safety and well being of the other students.
WATER DIAPERS: Children 3yrs and younger &/or not toilet trained MUST wear water diapers or they will not be allowed to swim. We will maintain a supply of these products at our office for your convenience. Please call office for further details.
PAYMENT: Full payment is due by the first day of class. If no payment has been received by that time, your credit card will be charged by the second week of the semester.
CANCELLATIONS/REFUNDS/CREDITS: When registered for a semester you will be held accountable for that first class. If you are not satisfied after taking the first class, a refund minus the cost of that first class will be given. This policy is only applicable for the first class; thereafter, the remaining cash value of the classes will be frozen for up to one year. There are no cancellations for the last two weeks of a semester, as we can not market your space at that point in a semester to another potential student.
NOTIFICATIONS: All canceled classes will be credited if made at least 72 hours prior to your next class, except on the last two dates of a semester at which time there are no cancellations accepted.
MAKE-UPS: If you miss a class, only one make-up is provided for students enrolled in 7 classes or more per semester. To schedule a make-up, call the main office no earlier then one day prior to the day you want to take the make-up (All make-ups must be done during the semester in which you missed the class. Make-ups will not be given during the first and last weeks of the semester). Your pool time, swim slot and teacher are reserved exclusively for you, for a specific number of weeks, Please plan accordingly, in order to attend all your classes. We appreciate if you would notify the office in advance if you intend on missing a class. If Take Me To The Water must cancel a class a make-up class may be scheduled. In the event an instructor is unable to teach, Take Me To The Water reserves the right to substitute without prior notice.
